Mounjaro Dosage Explained
How Mounjaro Dosage Works and When to Change It
Mounjaro (tirzepatide) is a weekly injectable medication for weight loss that uses a stepwise dosing approach to improve tolerance and long-term success. This page will guide you through the Mounjaro dosage progression, when to change your dose, and when to hold steady. A typical dose schedule looks like the following:
Mounjaro Dose Schedule
Titration Stage Dose Week 1–4 2.5 mg Week 5–8 5 mg Week 9–12 7.5 mg Week 13–16 10 mg Week 17–20 12.5 mg Week 21+ 15 mg (optional top dose) Note: Your specific schedule may vary depending on how your body responds, and your doctor’s recommendations.
What Is the Starting Dose for Mounjaro?
The Starting dose for Mounjaro is 2.5 mg for the first four weeks. This is the lowest Mounjaro dose and the one that you should start on to allow your body to adjust to higher doses with minimal side effects.
What Is the Highest Dose of Mounjaro?
The highest dose of Mounjaro is 15 mg, typically reached after gradual titration. This dose is tailored to those who tolerate the medication well and need maximum support for their weight loss goals.
Expert Insights
Dr. Charlotte Norton, our Chief Medical Officer, explains that medications like Mounjaro must always be started at the lowest dose due to their effect on gut function. Starting at a higher dose can cause unpleasant side effects such as nausea or bowel disturbances because the body hasn't had time to adapt. Starting low improves tolerance and still allows for effective appetite control and weight loss. Dose increases are gradual, based on how well the patient is doing.
Key Points:
- If weight loss is progressing and side effects are minimal, patients might stay on the same dose longer.
- If appetite suppression is insufficient or side effects persist, dose adjustments are made accordingly.
- It’s not a race, dosing is highly individual and should prioritise comfort, progress, and control.
- Many providers offer the medication, but don’t offer support from doctors or a clinical team.
- Simply eating less may lead to weight loss, but without the right nutrition, patients risk nutrient deficiencies.
At The Slimming Clinic, patients receive expert guidance and lifestyle support to ensure sustainable, effective results from their weight loss medication.
When to go up or down a dose?
Dose changes on Mounjaro should never be rushed. Each pen contains four fixed doses, one per week and you should complete a full month at each level before considering a change.
You might consider going up a dose on Mounjaro if:
- Your appetite isn’t fully under control.
- You’re not losing weight as expected.
- You’re not experiencing any persistent side effects.
Alternatively, it may be better to stay at your current dose or even go down a dose on Mounjaro if:- You're still experiencing nausea, bloating, or digestive issues.
- You feel unsettled even after four weeks on a dose.
The key to success is changing your dose based on how your body feels, not just following the calendar. It’s not a race to the top dose, some patients reach their goals on lower doses, while others titrate more quickly.

Do You Have to Increase Mounjaro Dosage?
Not at all. While most people follow a step-up dosing plan, it’s perfectly safe, and often beneficial for some to stay on the same dose longer. The right approach depends on your progress, side effects, and how in control you feel with your appetite.

Can I take 2.5mg from a 5mg Mounjaro pen?
There’s growing mention online about so-called "Mounjaro hacks". One of these hacks includes buying a 5mg pen and injecting 2.5mg of it because you may have bought the wrong pen, or you want to change dose.
Our Chief Medical officer, Dr Charlotte advises against trying to manually dose 2.5mg from a 5mg pen, as this method is uncontrolled, untested, and unreliable. Techniques like counting "clicks" are not fail-safe and may result in injecting the wrong amount.
However, if you've stored your 5mg pen correctly in the fridge, it can still be used later when you're ready to move to that dose. The pen remains effective until its expiry date as long as it hasn’t been kept at room temperature.
Proper storage, clean needles, and using the correct pen for the intended dose are key to staying safe and getting the full benefit of your medication.
What Is The Mounjaro Golden Dose?
Another "hack" which is being talked about is the “golden dose” of Mounjaro, where individuals attempt to extract an unofficial fifth dose from pens which are primarily designed to deliver only four doses. While this might sound like a clever hack, it’s actually a very risky practice that raises serious safety concerns.
Tampering with medication dosing or device functionality can compromise both your treatment and your health. It’s important to stick to clinically approved usage and dosing schedules.

Staying on Track: What Happens After You Reach Your Goal?
Reaching your target weight is a major milestone, but it’s not the end of the journey. Many patients continue treatment at a lower dose to help maintain their results and prevent unwanted rebound weight gain.
Adjusting your dosage gradually and under clinical guidance is key to long-term success.
